The Office of the University Registrar (OUR) collects certain information outside of the system through the web forms listed below. Please use these forms to send specific requests to OUR for approval and creation.

Designated school officials with authority to approve a student’s return from suspension should complete and submit the Request for Student to Return from Suspension form only after all required approvals and any applicable sanctions have been fulfilled. The Office of the University Registrar expects school registrars to communicate directly with the student regarding their return timeline and next steps. Once the form is submitted, OUR will complete the request to return within 1–3 business days. If additional information is needed, OUR will contact the requestor directly. Formal confirmation will be sent once the student’s record has been updated.
Researchers requesting to use student data, without student consent, in their studies must meet specific conditions under FERPA. Complete this form and submit with your research proposal, or any questions about this process, to studentdatarequests@email.washu.edu. Inquiries and requests will be acknowledged within 1-2 business days and reviewed as soon as possible. Depending upon the completeness and complexity of the request, we may respond with questions for the researcher.
